BIST19 : Surveiller l'apparation des séquences H5N1 résistantes au Tamiflu®


 Objectif : repérer chaque semaine si une nouvelle séquence de virus H5N1 aviaire résistant au Tamiflu est retrouvée et dans quel pays…..


Le papier de N.Engl Journal of Medecine (http://content.nejm.org/cgi/content/full/353/25/2667) décrit une mutation dans la séquence du gène de la neuraminidase (NA) du virus H5N1 présent chez un patient qui était résistant au traitement par l’oseltaminir (Tamiflu®).

CONCLUSION     

Un virus isolé à Hong Kong en 2001 est partiellement résistant…

Mieux vaut prévoir un traitement avec 2 composés distincts (Tamiflu et Amantadine par exemple…)

Use of antivirals in H5N1 infection

The safety and clinical effectiveness of NIs against the H5N1 strain is unknown, and currently published data include a limited number of observational cases from Thailand and Vietnam. The role of NIs in the management of seasonal influenza, and the safety profile of these drugs, is well known. The NIs were developed and licensed in the setting of seasonal influenza and geared towards identifying the minimal effective dose and regimen duration.
