Changes to Tecfa topics:

- Changed all xref to indextermref (because they Map FM's empty Crossref exports)
- (query-replace "desc.cnt" "para.cnt" nil) because it contains indextermref

NOTE: In FrameMaker indextermref is internally translated to XRef. Hated to change
all the XRefs that were already in the Catalog ....
So you REALLY must use the dita.rules file and declare it with Frame !!
